Kalman vs Freshpaint
Freshpaint is a broad marketing data platform built for large healthcare organizations. Kalman is built for one job: getting DTC telehealth brands a clean, valuable conversion signal on Meta, from the patients who actually get approved and refill. If you run a telehealth brand or the agency behind one, this page shows where that difference matters.

Key takeaways
- Kalman optimizes Meta on what telehealth brands are paid for: approved patients and refills, with a lifetime value attached. Freshpaint's public pages describe website and health-system integrations, not telehealth-platform outcomes.
- Kalman connects to the telehealth stack (Sylaris OS today, Bask Health, Telegra and MDI by webhook) and Stripe. Freshpaint's integrations page lists systems like Epic MyChart and Experity instead.
- Kalman publishes its prices and lets you scan your site without a call. Freshpaint's three plans are priced through sales.
- Kalman sends Meta an allowlist and logs every event it sent, so you can show a client or their counsel exactly what left.
- Freshpaint covers far more ground: a consent manager, audiences, session replay and destinations like DSPs and data warehouses. If you need all of that across a hospital system, it's the broader tool.
What Kalman actually does
Kalman is a signal layer for direct-to-consumer telehealth brands that buy patients on Meta. It does three jobs, and it does them for the funnel telehealth brands actually run: an ad, a landing page, an intake, a checkout, a provider decision and then refills.
1. It tells Meta who actually became a patient
A browser pixel sees page views and form fills. It never sees what happens after checkout, because that happens on the telehealth platform: the provider approves the patient, the pharmacy ships, the refill bills next month. Kalman takes those outcomes straight from the platform (Sylaris OS natively today; Bask Health, Telegra and MDI through a webhook) and from Stripe, and sends them to Meta server-side through the Conversions API: purchases, refills as repeat purchases, and approvals when your consent setup allows it.
Each conversion carries a value you choose: the order value, a projected lifetime value per program, or the average lifetime value Kalman calculates from your real refills. Meta's value optimization then learns to find patients who stay, not just people who fill in a form.
2. It keeps health details out of what Meta receives
Every outbound event is built from an allowlist: the event name, the time, the value, hashed identifiers (email, phone, your patient ID), Meta's click and browser IDs, and your site's root address. Page paths that name a condition or a medication, product names and anything typed into a form never leave. Custom event names are checked against a list of condition and medication terms before they're allowed.
You don't have to take that on trust. Kalman keeps a log of every event it sent, and you can preview any event before a destination goes live. Every customer gets a BAA before any data flows.
3. It shows you what a patient really costs
Kalman's own analytics run inside Kalman, on your first-party data, so you don't need a third-party analytics tag on health pages to see the funnel: traffic by source, step-by-step intake drop-off, and acquisition with your Meta spend imported, so you see cost per purchase and per approved patient by campaign. That's the number a growth lead actually manages to.
Built around the telehealth stack
- First-party by default. The tag runs on your own subdomain, keeps ad click IDs in a first-party cookie and carries them to your intake, so the approved patient is tied back to the ad that brought them.
- A free scan before you talk to anyone. The scanner shows what each tracker on your site sends today, page by page, and what to fix first.
- Published prices. Signal is $499 a month and Signal + Attribution is $999. Agencies pay a $0 platform fee and then per brand.
- Built by an operator. Kalman's founder built NimbusRX and scaled Sylaris OS, so the product is designed around how telehealth funnels and their webhooks really behave.
What Freshpaint offers
By its own description, Freshpaint is a healthcare marketing data platform. Its pages describe a consent manager with state-based rules, tracker monitoring, audiences, attribution, session replay and integrations that reach ad platforms, demand-side platforms and data warehouses. Its customers and examples are mostly large organizations: hospitals and health systems, payers, urgent care, dental and specialty providers, pharma and device makers, and virtual care.
It sells three plans (Comply, Perform and Scale) without published prices, and says most customers finish implementation in about three weeks. It describes its tracking as covered by a BAA or DPA.
Where Kalman and Freshpaint really differ
The conversion you optimize on
This is the biggest difference. For a telehealth brand, a form fill or even a checkout isn't the outcome: plenty of checkouts are declined by the provider, refunded or never refilled. Kalman takes the approval, the order and each refill from the telehealth platform and sends them to Meta with value, so campaigns learn from patients, not leads. Freshpaint's public pages describe collecting website events and connecting healthcare systems such as Epic MyChart and Experity; we couldn't find telehealth approvals or refills described there.
Telehealth platform integrations
Kalman is built around the platforms DTC telehealth brands run on. Sylaris OS sends signed events to Kalman natively, Bask Health, Telegra and MDI connect through a webhook today, and Stripe connects directly. Bask Health, Telegra, MDI and Stripe aren't listed on Freshpaint's integrations page.
Value and lifetime value
Kalman lets each conversion carry a projected lifetime value per program, then replaces your estimate with the average lifetime value it calculates from real refills. That's what makes Meta's value optimization worth turning on for subscription treatments. We couldn't find how Freshpaint values conversions on its public pages.
Pricing and how you buy
Kalman's prices are on the pricing page, the scan is free and instant, and setup is done with you in a short call. Freshpaint asks you to talk to sales for every plan. For a scaling brand or a small agency, that's the difference between trying it this week and going through procurement.
Scope
Freshpaint does more things. Kalman does one thing deeply: the Meta signal for telehealth, with first-party analytics so you can see cost per approved patient. If your plan is one platform for every channel and every team in a health system, Freshpaint is built for that. If your plan is better Meta results for a telehealth brand this quarter, Kalman is.
Which one fits your business?
Choose Kalman if you:
- run a DTC telehealth brand (GLP-1, TRT, men's or women's health, hair, peptides) or the agency behind one;
- buy most of your patients on Meta and want it optimizing on approvals and refills, not form fills;
- run on Sylaris OS, Bask Health, Telegra, MDI or Stripe;
- want published prices, a free scan and setup in days, not weeks.
Freshpaint may be the better fit if you:
- are a hospital, health system, payer or other large organization with many sites and teams;
- need integrations with systems like Epic MyChart, or a consent manager, audiences and data-warehouse destinations from one vendor;
- need Google Ads conversions today rather than in a beta.
FAQ
Is Kalman a Freshpaint alternative?
For DTC telehealth brands running Meta, yes. Kalman replaces the Meta tracking part of the job with a signal built on approvals and refills. It isn't a full marketing data platform, so if you rely on Freshpaint for many other destinations, compare those separately.
How much does Freshpaint cost?
Freshpaint doesn't publish its prices; its three plans are quoted by sales. Kalman's prices are published on the pricing page.
Can I switch from Freshpaint to Kalman?
Yes. Kalman installs alongside what you have: one first-party tag and a webhook from your telehealth platform. You preview every event before the Meta destination goes live, then remove the old Meta setup.
Does Kalman have a consent banner?
No. Kalman works with the consent tool you already use. If you need a consent manager from the same vendor, Freshpaint includes one.
Do I get a BAA?
Yes. Every Kalman customer signs a BAA before any data flows, on every plan.
The bottom line
Freshpaint is a broad platform for large healthcare marketing teams. Kalman is the focused choice for DTC telehealth: it feeds Meta the approvals and refills your business runs on, with a lifetime value attached, keeps health details out of what Meta receives, and publishes its prices.
